These Terms outline how we provide services at MH Pathways. By booking an appointment, submitting a referral or using our website,
you agree to these Terms of Service.

About MH Pathways

MH Pathways is a multidisciplinary mental-health practice providing care through:

  • Mental Health Nurse Practitioners

  • Psychiatrists

  • Psychologists

  • Social Workers

  • Occupational Therapists

  • Dietitians

We offer in-office, telehealth, telephone, and community-based support.

Our goal is to provide safe, ethical, person-centred care delivered with professionalism, compassion and respect.

Appointments & Bookings

Making a booking

Appointments can be made through:

  • Online booking links (when available)

  • Phone or email enquiries

  • Referral forms (NDIS or Psychiatry Referral)

Some services may require a GP referral, medical information or background documents before an appointment can be confirmed.

Telehealth

Telehealth appointments may be offered via secure video or telephone.

It is your responsibility to ensure:

  • a private, safe environment

  • a stable internet or phone connection

  • any required documentation is available

Telehealth may not be appropriate for all clinical situations.

Cancellations

Cancellations or rescheduling require 24 hours’ notice unless otherwise specified.

Late cancellations or missed appointments may incur a fee at the clinician’s discretion.

Fees & Payment

MH Pathways provides services to:

  • Private paying clients

  • Medicare bulk-billed clients (where applicable)

  • NDIS participants (self-managed or plan-managed)

  • DVA and other funding schemes (when applicable)

Fees vary by clinician and service type.

You will be informed of any fees prior to your appointment.

Invoices must be paid in accordance with the terms provided.

Outstanding accounts may affect future bookings.

Clinical Care & Responsibilities

Your responsibilities

To support safe and effective care, you agree to:

  • Provide accurate and up-to-date information

  • Advise us of any risks, safety concerns or changes

  • Attend appointments on time

  • Follow emergency instructions if required

Our responsibilities

Our clinicians will:

  • Provide care in line with their professional scope

  • Maintain confidentiality (as outlined in our Privacy Policy)

  • Obtain your consent for treatment

  • Communicate clearly about your care plan

  • Work collaboratively with other treating professionals (with consent)

Collaborative care

With your consent, we may share relevant information with:

  • Your GP

  • Psychiatrist or psychologist

  • Support coordinator

  • Other treating clinicians

This helps ensure cohesive, safe and coordinated care.

Risk, Safety & Emergencies

We do not provide crisis or emergency services.

If you are in immediate danger or experiencing an emergency, contact:

  • Your local mental-health crisis team

MH Pathways may, in rare circumstances, need to act to protect you or others from serious harm. This may involve contacting
emergency services or appropriate supports.

Website Use

When using our website, you agree not to:

  • misuse forms or appointment links

  • upload harmful, abusive or inappropriate content

  • interfere with website functionality

  • attempt to access restricted or unauthorised systems

We may update website content at any time.

Referrals

MH Pathways accepts:

  • NDIS referrals

  • Psychiatry referrals

  • GP referrals

  • Self-referrals

  • Professional referrals from support coordinators or allied health

Different referral types may require different documentation.

We reserve the right to redirect referrals that are outside our clinical scope.

Consent to Treatment

By attending an appointment, you consent to:

  • Assessment

  • Diagnosis (when applicable)

  • Treatment planning

  • Clinical documentation

  • Sharing relevant information with other treating professionals (with your permission)

You may withdraw consent at any time.
